The End of Life Strategy is concerned with ensuring patient choice and quality in the care
that they wish to receive at the end of their lives. In order to deliver this there are a
number of key components we need to consider:

Identifying Patients approaching the end of their lives. This involves asking the GSF question
as to whether you would be surprised if this patient were to die in the next 6-12 months.

Having discussions about future care that a patient and their family consider they would want.
This is advance care planning and may lead to completion of appropriate paperwork.

Documenting and communicating (with the patient consent) about what a patient's wishes are
for end of life care.
A wide range of healthcare professionals need to be aware of the changes that are occurring in
delivering excellent patient-centered care. At Weston Hospicecare we have put together this DVD
to help inform and educate a broad range of healthcare professionals as to the bigger picture
and the impact it can have on patients and families.
This DVD is not designed as a "how to do it" guide, but as a general introduction to how practice is
changing and the effect this can have. It features palliative care doctors and nurses as well as patients
and relatives who have been involved in advance care planning. We are using the DVD as an introduction
to our education sessions to a broad range of healthcare professionals, which is then followed by sessions
tailored to the needs of the individual group.
