Making a referral

We have a variety of services on offer at Weston Hospicecare.

At Weston Hospicecare, we provide high quality end-of-life care to residents of Weston-super-Mare and the surrounding areas.

Depending on the reason for referral, an appropriate member of our team will make contact with the patient and agree an appointment time. This may be a clinic appointment (which is an appointment at our hospice with an appropriate member of the professional hospice team – not unlike a GP appointment) or a community visit.

If the referral is urgent, we have special provisions in place. A member of the team will make telephone contact within two working days at most. We are generally only able to take admissions for our In-Patient Unit from Monday-Friday up until 3 p.m.
