We have a variety of services on offer at Weston Hospicecare.
At Weston Hospicecare, we provide high quality end-of-life care to residents of Weston-super-Mare and the surrounding areas.
Referral to Weston Hospicecare is appropriate for patients who meet the following criteria:
- They have life-limiting disease with complex associated problems that may be physical, psychological, social, emotional or spiritual in their origins.
- Due to the level of complexity and/or intensity of their illness, their needs cannot solely be met by other services/care providers at this time.
- They live within and / or are registered with a GP within our coverage area (see map).
- Both the GP and patient/advocate agree to the referral.
Referrals are welcomed from consultants, GPs, clinical nurse specialists, district nurses and community matrons. If the GP is not the referrer, please be sure to advise the patient’s GP of your referral.
Depending on the reason for referral, an appropriate member of our team will make contact with the patient and agree an appointment time. This may be a clinic appointment (which is an appointment at our hospice with an appropriate member of the professional hospice team – not unlike a GP appointment) or a community visit.
If the referral is urgent, we have special provisions in place. A member of the team will make telephone contact within two working days at most. We are generally only able to take admissions for our In-Patient Unit from Monday-Friday up until 3 p.m.
